1. Download the Product Order Form

2. Complete the Product Order Form

   • Write the Item Number using Deb's Threadz's Menu.

   • Include any customizations for your item(s).

   • Blank forms are available at CCS's front desk and at Deb's Threadz pop-up events.

3. Submit it to Deb's Threadz

You have several options:

   • Leave the completed order form with CCS's front office

   • Hand-deliver the completed order form to Deb's Threadz at a pop-up event

4. Deb's Threadz will email you to confirm receipt and discuss the next steps (payment, delivery, etc.).

5. Your items will be delivered to Colton Christian School's front office.
